Community Health Nursing
High Intensity Daily Personal Activities
Providing support by a worker with additional qualification and experience to assist or supervise daily personal tasks of a participant with complex needs in order to enable the participant to live as independently as possible.
Participation in Community, Social and Civic Activities
Assisting participants to participate actively in their community which includes supporting participants during activities and developing participants’ ability to join activities. Activities such as attending personal development courses, joining a social group, taking a holiday, library visit, cinema, community events and joining sporting clubs.
Assistance with Travel / Transport Arrangements
Assisting participants in their transportation requirements to allow them to be mobile and achieve a sustainable and independent life while increasing involvement in the community. This may include transport assistance if the participant cannot take public transport. This may also involve helping you learn how to use public transport, transport provision when you need to attend appointments, grocery shopping, picking up prescriptions from chemist, attending events, visiting parks, etc.
Assistance with Daily Personal Activities
Assistance provided by a support worker while promoting participant’s independence such as the following: personal hygiene, including showering, bathing, oral hygiene, dressing and grooming, toileting, bladder and bowel management, eating and drinking, mobility getting in and out of bed and attending appointments.
Household Tasks
Support provided includes assistance with domestic chores such as cleaning, dishwashing, laundry, meal preparation, home and garden maintenance with focus on training skills and independence.
Future Offerings
Specialist Positive Support
This intervention assists customers to reduce the risk of challenging or harmful behaviors and support positive outcomes. Support may include teaching communication strategies, changing aspects of environment, providing access to fun activities and community connections, and skills building.
Assistance in coordinating or managing life stages, transitions and supports
This includes short and long term supports that helps strengthen the participants’ ability to coordinate their supports and to assist them to live at home and participate in community such as support connection, assistance with accommodation and tenancy obligations, life transition planning including mentoring, peer support and individual skill development.
Assistance with daily life tasks in a group or shared living arrangement
We help by assisting or supervising participants with daily activities of living in a group setting or shared living arrangement in accordance with each participant’s needs. The support is done with focus on training skills and promoting independence.
Group and Centre-based Activities
We provide individualised and group activities that promote socialisation and foster capacity building.
Development of Daily Care and Life Skills
Developing daily living and life skills can be helped by training and development that increase one’s ability to live independently. Training is undertaken by the participant/and with the support worker and can be provided individually or in groups. Activities might include skills in daily life activities, communication, and social skills, problem-solving, and managing the funding of supports.
Exercise Physiologist and Fitness
Provisions of supports to encourage the participants with their fitness goals by giving them strategies to achieve these goals.
